Midwife Model of Care:
The Midwives Model of Care is based on the fact that pregnancy and birth are normal life processes. The Family Practice Midwifery wellness plan includes:
Breastfeeding Support: We will provide support and education to you as you and your baby begin your breastfeeding relationship. |
Family Practice Midwifery Fees: The expenses involved in midwifery are substantial and typically not obvious to the families we serve. While midwives almost always agree that midwifery is far more about the love of our work than any real financial gain, the service we provide must pay for the cars, gas, phones, supplies, equipment and time we put into it. I recognize that every family has different needs and resources and I will attempt to work with each family according to these needs. I offer a sliding fee scale range but please be aware that the base fee is calculated at current minimum wage.
I work with a sliding scale of $2,500 to $5,000 with the understanding that the fee for my assistant is taken from that amount. I do not ask for any financial documents, I have faith that most people intend to be honest. I ask that you consider your own income and determine where to place yourself on this scale.
There is a $500 good-faith deposit due with your decision to hire me as your midwife. This secures a date in my practice for your birth. This fee is non-refundable.
Total cost of the birth fee is due prior to the birth of your baby and is also non- refundable.
This fee covers the standard care of the American College of Obstetricians:
Monthly visits from your Initial Consultation to approx. 30 weeks
An appointment at approx. 33 weeks
Every 2 weeks thereafter until approx. 36 weeks
Weekly visits until the birth of your baby.
These visits include: urinalysis, hemoglobin, blood sugar, evaluation of mother and baby and referral to MD if necessary.
The fee includes the birth with a newborn exam included and any blood typing necessary for Rh neg mothers.
Rhogam charge NOT included-price varies. Prenatal and Comprehensive Metabolic lab panel charges
NOT included.
Your postpartum visits include:
Approx: 24 hours
Approx. 3 to 4 day postpartum
Approx. 14 day postpartum
Approx. 4 to 6 week postpartum.
Standard lab work is available during your visits: urine, blood sugar, hemoglobin and evaluation of mother and baby.
Any additional lab work necessary is offered at the fee we are charged for each test according to the local labs we use.
Your birth certificate application and Newborn Screening for the baby are also included in the fee however, the Newborn Screening costs are $109 for those who can afford to pay for this service. Also, you are charged by the state for your actual birth certificate. Hearing screening is also provided for a minimal cost of $10.
If there is to be a transfer to the hospital before, during or after the delivery, if your baby is born before the arrival of the midwife or if you delivery without notifying midwife before hand, will not result in a refund.
You are free to use as many or few of the visits as you wish. However, it is important for you to understand that there is no discount for less visits. If you wish addition consultations with other practitioners, this is your expense and does not reduce my fee.
